Advanced Care Planning (ACP)

When we think about planning for our future, we often focus on our careers, education, finances, retirement…and we sometimes forget that we also need to have conversations about Advance Care Planning: what are your wishes and values that influence the kind of health and personal care you would want in the future if you were unable to speak for yourself. In this workshop we will:

· review health care consent in Ontario;

· understand the role of and identify your Substitute Decision Maker(s);

· explore your wishes, values and beliefs about health and personal care;

· offer tips for having ACP conversations with the people who are important to you; and,

· share resources to support you in your planning.

 

This is a workshop, therefore we heavily encourage you to turn your cameras and mics on and be a part of the discussion!

 

About the Presenter: Nadine Valk

Nadine is a full-time coach based in Ottawa, with a focus on working with caregivers, care providers and care teams. In  her work as a coach, She draws on over 30 years of experience in health, community, and palliative care (most recently as Director of Education for Hospice Palliative Care Ontario and as the Director of the Champlain Hospice Palliative Care Program). She has a master’s degree in public administration with a focus on health policy and expertise in program development, change management and education. As a certified Integral Professional Coach™ with additional training and certification in team coaching, facilitation and implementation science, Nadine’s goal is to ensure that people she works with feel seen, supported, and connected, so that change is integrated in a way that is both practical and purposeful. As a mindfulness teacher and Director of the Mindfulness Informed End of Life Care Program at the University of Toronto School of Continuing Studies, her goal is to weave compassion into every aspect of her work, and cultivate a supportive environment for meaningful transformations in how we care for ourselves and one another.
